Abstract

Notwithstanding the implementation of reliability engineering programs by companies, it is not uncommon to observe many instances of low field reliability. This paper briefly describes the essence of reliability engineering, which has prevention of failure as primary objective. Possible reasons for the apparent failure of reliability engineering, especially as practised by the defence industry, are given. It argues that incorrect practices are often applied, frequently performed by incorrect people in the organisation, and at the incorrect time during the product of system life cycle. The value of reliability engineering as specialty area of systems engineering can be improved by learning from best practices used by industry leaders.
